On the other end of the spectrum, residents of Scotland County have the lowest median income, and rank No. 99 in health outcomes.

Those gathering the data behind the rankings say the reports can be used to target areas of improvement in communities lagging behind the rest of the state. "It's not okay to continue to do the same things we've done for years".

The foundation examines health outcomes, access to medical care and socioeconomic factors such as child poverty rates and single-parent households.
